Objective: To connect individuals with developmental disabilities and their families to state agencies and community services.

Strategy: To assist individuals with developmental disabilities and their families to complete the allocation for the iBudget Home and Community Services Waiver and work with the Agency for Persons with Disabilities to secure funding for needed services.

Outcomes: Individuals with developmental disabilities and their families receive funding for needed services.

This service is provided for adults with developmental disabilities and their families living in south Lake County. We provide assistance with completing the Florida Medicaid Waiver application package; offering support and guidance while navigating the application process with the Agency for Persons with Disabilities.
